If you’re sending us a tax payment, complete Form OR-40-V with your name and Social Security number (SSN) (and your spouse’s name and SSN, if making a joint payment), address, and contact phone. Include the beginning and ending dates for the tax year. Enter the payment amount from line 3 of the tax payment worksheet, and check the “Extension payment” checkbox. Make your check or money order payable to “Oregon Department of Revenue.” Write the last four numbers of your SSN and “2019 Form OR‑40-V” on your check or money order. To help us process your request faster, use blue or black ballpoint ink. Don’t use gel ink, or red or purple ink, and don’t post date your check. Form 40-EXT is an Oregon Individual Income Tax form. The IRS and most states will grant an automatic 6-month extension of time to file income tax and other types of tax returns, which can be obtained by filing the proper extension request form. Obtaining an extension will prevent you from being subject to often very large failure-to-file penalties. However, in most cases this extension does not exempt you from the requirement to pay any tax owed in full by the return's original filing date.

About the Individual Income Tax

The IRS and most states collect a personal income tax, which is paid throughout the year via tax withholding or estimated income tax payments.

Most taxpayers are required to file a yearly income tax return in April to both the Internal Revenue Service and their state's revenue department, which will result in either a tax refund of excess withheld income or a tax payment if the withholding does not cover the taxpayer's entire liability.
