PA-41 Payment VoucherPa-41 V 06-19 (FI) Pa-41 PaymeNt VOuCheR PA-41 PAYMENT VOUCHER (PA-41 V) Use the 2019 Form PA-41 V with a payment of tax owed with a 2019 PA-41, Fiduciary Income Tax Return. Do not use this voucher for any other purpose. Follow the instructions below. ● City – Print the name of the city. For foreign addresses, the bottom line of the address should show only the country name,written in full (no abbreviations) and in capital letters. ● State – Print the two-digit state abbreviation. ● ZIP – Print the five-digit ZIP Code. FEDERAL EMPLOYER IDENTIFICATION NUMBER (FEIN)/ SOCIAL SECURITY NUMBER (SSN) ● FEIN - enter the fiduciary’s (estate or trust) nine-digit FEIN without a hyphen followed by “F.” ● SSN - If the estate has not received its FEIN from the IRS, enter SS followed by the first seven digits of the decedent’s Social Security number without the hyphens followed by “F”. ● NO FEIN – if the trust has not yet received its FEIN from the IRS, write “APPLIEDFR” in the space provided for the FEIN. FIDUCIARY PHONE NUMBER Print the fiduciary’s (estate or trust) 10-digit phone number with area code including hyphens. PAYMENT Carefully enter the amount of the payment. Make the check or money order payable to the PA DEPT. OF REVENUE. Please write on the check or money order: ● the estate’s or trust’s FEIN (including “F”); ● "2019 PA-41 V"; and ● the daytime telephone number of the estate’s or trust’s representative. CHECK DIGIT Print the first two letters of the last name of the fiduciary (estate or trust). The department will need the FEIN if the check becomes separated from the PA-41 V. The department will need the FEIN to accurately apply the payment. NAME AND TITLE OF FIDUCIARY ● Print the last name, first name and middle initial of the fiduciary (estate or trust). Example: ● Estate – SMITH JOHN T ESTATE OF ● Trust – JONES MARY S TRUST MAILING ADDRESS If filing a paper return, please remit payment with the return. If filing electronically, please use the following address to mail a payment to the department: ADDRESS OF FIDUCIARY ● First and Second Line of Address – Print the fiduciary’s (estate or trust) complete street address. Form PA-41 PA-V is a Pennsylvania Corporate Income Tax form. Payment vouchers are provided to accompany checks mailed to pay off tax liabilities, and are used by the revenue department to record the purpose of the check and the SSN/EIN of the taxpayer who sent it. Many states recommend that taxpayers also write their social security number and the purpose of the payment on the check itself, in case the accompanying payment voucher is misplaced or destroyed.
About the Corporate Income Tax
The IRS and most states require corporations to file an income tax return, with the exact filing requirements depending on the type of company.
Sole proprietorships or disregarded entities like LLCs are filed on Schedule C (or the state equivalent) of the owner's personal income tax return, flow-through entities like S Corporations or Partnerships are generally required to file an informational return equivilent to the IRS Form 1120S or Form 1065, and full corporations must file the equivalent of federal Form 1120 (and, unlike flow-through corporations, are often subject to a corporate tax liability).
