Please note that each form is year specific. To prevent any delay in processing your return, the correct year’s form must be submitted to the Department of Revenue Services (DRS). Purpose: Complete Form CT‑1040V if you filed your Connecticut income tax return electronically and elect to make payment by check. You must pay the total amount of tax due on or before April 15, 2021. Any unpaid balance will be subject to penalty and interest. Pay by Mail: Make check payable to Commissioner of Revenue Services. To ensure payment is applied to the correct account, write “2020 CT‑1040V e-file” and your Social Security Number (SSN), optional, on the front of the check. Sign the check and paper clip it to the front of the voucher. Do not send cash. DRS may submit the check to your bank electronically. Return the voucher below with your payment. Mail to: Department of Revenue Services State of Connecticut PO Box 2921 Hartford, CT 06104-2921 Do not submit a paper copy of your Connecticut income tax return with this voucher. Other Payment Options A. Pay Electronically: Visit portal.ct.gov/TSC to use the Taxpayer Service Center (TSC) to make a direct tax payment. After logging into the TSC, select the Make Payment Only option. Using this option authorizes DRS to electronically withdraw a payment from your bank account (checking or savings) on a date you select up to the due date. As a reminder, even if you pay electronically, you must still file your return by the due date. Tax not paid on or before the due date will be subject to penalty and interest. B. Pay by Credit Card or Debit Card: You may elect to pay your 2020 tax liability using a credit card (American Express®, Discover®, MasterCard®, VISA®) or comparable debit card. A convenience fee will be charged by the service provider. You will be informed of the amount of the fee and may elect to cancel the transaction. At the end of the transaction, you will be given a confirmation number for your records. There are two ways to pay by credit card or comparable debit card: • Log in to your account in the TSC and select Make Payment by Credit Card; or • Visit www.officialpayments.com and select State Payments. Your payment will be effective on the date you make the charge. Write your SSN (optional) and “2020 CT‑1040V e-file” on your check. Make check payable to Commissioner of Revenue Services. Do not use staples. Do not submit a paper copy of your Connecticut income tax return with this voucher. Separate here and mail voucher to DRS. Make a copy for your records. Form CT-1040V is a Connecticut Individual Income Tax form. Payment vouchers are provided to accompany checks mailed to pay off tax liabilities, and are used by the revenue department to record the purpose of the check and the SSN/EIN of the taxpayer who sent it. Many states recommend that taxpayers also write their social security number and the purpose of the payment on the check itself, in case the accompanying payment voucher is misplaced or destroyed.
About the Individual Income Tax
The IRS and most states collect a personal income tax, which is paid throughout the year via tax withholding or estimated income tax payments.
Most taxpayers are required to file a yearly income tax return in April to both the Internal Revenue Service and their state's revenue department, which will result in either a tax refund of excess withheld income or a tax payment if the withholding does not cover the taxpayer's entire liability.
